Smoking cessation management plans for patients receiving cardiology or oncology care

Smoking cessation management plans have been developed for patients receiving cardiology or oncology care, in response to a need identified through consultation with health professionals. These plans promote a continuity of care from specialists, following a discussion with their patient, to the patient's other health professionals. They record the patient's smoking status with a management plan including behavioural support (with Quitline referral link), and/or pharmacotherapy.

The plans also provide next steps for the patient to take, including engaging with Quitline, their pharmacist (if medication is required), and importantly, with their GP to ensure the specialist’s advice is conveyed to the GP and incorporated in health plans.

These interactive plans can be completed online and/or printed to give to patients.
